I want to stop to use Jotform but Jot form send invoice for October fee.How I stop this

I use Jot form on October 2016 1 month but On November Jot form send invocie on November I want to stop to pay this .Please help.

I can't remember user and password also.

What is your JotForm account username or the email address you have used in your account profile? I am unable to find any details with any of your email addresses napit2011@hotmail.com or napitchaya@strek.co.th 

Alternatively you can also let us know any of your form ID or the webpage URL where you have embedded your form and we will take a look.
