I logged in to your account but I wasn't able to replicate the issue. In My Forms, I selected the form and then click the "More" button. After that I chose the "View Form" option and the form appeared correctly. Here's a screen capture:
I would recommend you disabling any pop-up blockers. You may also disable AdBlock or uBlock temporarily. If the issue persist, please try using another browser just to check if it will work or not.
