Can I create a form for technical assistance?

I would like to create a form that's fillable for me to complete during technical assistance. It would need check boxes but also lists of areas and text boxes for me to add comments.

If you want to update the submissions on a later date and add comments, that's possible using the edit URL. http://www.jotform.com/help/40-How-to-Let-Users-Update-Their-Form-Submissions-at-a-Later-Date

You can also edit the submissions in your form submissions page.
