Can't see my jotform from yesterday and displaying any field?

Why I can't see my Jotform from yesterday? Can anyone plz help?

Your form is setup to display if an authentication word is entered, however, nothing is showing nor the field that asks for this authentication word, because you have also included it into the set of fields that will remain hidden until the correct word is entered.

 So, simply edit your condition and deselect that field by pressing ctrl + left click:
