Widgets are not displaying when the form is embedded using Dreamweaver

In the mean time,.. I have issue in the incorporating the form into dreamweaver.  - see :

important boxes are missing such as the Slots Available, as well as the Terms and conditions. 

I see that the form is embedded using the source code of the form which may be conflicting with the other scripts on the web page. Could you try embedding the form using iframe embed code?
