Do you have form that can be added to outlook email?

we currently use the outlook signature, but it is not a true form.  We have various processes in our office team that fillable email forms would be helpful

You can send a direct link of your JotForm form through email, but your users will have to open that link in a web browser and fill out the form that way.

The form cannot be filled out directly within the email they receive.
