The text in some of my forms does not show on the submissions

Once people fill out and submit the forms that I have like this one, the main body of the form will not show up so it doesn't show that they are agreeing to anything or what they are submitting.  Is there something that I am missing on the form?  I am using it as a release/agreement.  

Are you referring to the text fields on your form? If so, unfortunately, these text fields will not be displayed automatically in the emails. If you need to have the text with the agreement in the emails, it is required to have the text manually. Please copy the text that you want to display in the email.
