How can I have the fields in the form remain hidden until an option is selected?

I have been trying to make fields under  PERSONALISED FORM visible to customers when they press YES or PAY.  I want these fields to remain hidden when they click NO or DON'T PAY.  PLEASE HELP OUT

Setup the conditional logic to SHOW fields when Pay is selected. The fields will remain hidden until YES is selected. You do not need the opposite HIDE condition.
