When I hit the "Test email" button, I do get an email in the auto-responder format. However when I fill out the form and include a valid email address I get only the notification email but no auto response email to the address entered on the form.
Been trying all morning to get this to work!

Webpage you provide is unavailable, so I have tested only form that you have on your account. When I submitted your form I received Autoresponder email.
I have checked email history log on your account and I found some failed Autoresponder emails. I noticed that all failed emails were sent from your own SMPT email address. Most likely you did not setup your SMTP credentials right.
Currently Sender Email of your Autoresponder email is firstname.lastname@example.org. Considering that now your Autorespinder email is working correctly, I would suggest you to leave email@example.com as Sender Email.

Unfortunately it is still not working for me. I tried two additional times, I do get the Notification email letting me know there's been a submission, but no email is sent to the email entered in the form. I even tried my hotmail email firstname.lastname@example.org and still nothing was received at that address either.
You are correct in an attempt to get this to work I tried setting up my own SMTP settings but that didn't work either.
I may try making a new form perhaps it's something corrupt within this particular form?

I have tested your form and the embedded link you have shared us; however, I cannot replicate the problem, I have submitted the form and as soon as the form was sent the email auto-responder was sent to me, here's a screenshot:
I have checked the email address you provided and I cannot find it in our bounce list, one of the reasons why emails do not send, but I would suggest you to check other email addresses you're using and if you find them simply removing should help to start receiving emails again: How-to-Prevent-Email-Bouncing-Related-Issues
Could you please also try submitting your form again?
Please if you're still unable to get your email auto-responder do let us know, please also test with another email address to see if the email is sent.
