Having issues customizing a form template from the Templates page

 everything I try to customize breaks this design -- I have to hack the hell out of it!

We would like to apologize for any inconvenience. It seems that the form you've used as a template is a form contributed by another user.

Forms submitted as a template by other users are still saved in their account and they are the only one that can update/edit it.
