Why did my form fields disappear?

I spent an hour creating a form yesterday. Today all the fields have disappeared.

What happened? I'm not likely to keep using this solution if I can't trust that the form will not change.

I was looking at my form, I clicked the templates to see what templates you have to offer, clicked the back button to go back to my form, and all the fields were gone.

Can you confirm to us that you click the Save button before going to the themes section and going back to the My Forms page? Also, you should use the back button in the form builder, not the back button in your browser. I've checked your session history, I can't seem to find the timeframe where you've lost the changes.

Also, you can revert back your forms to the previous version where the form fields are intact through the revision history section. Please check https://www.jotform.com/help/294-How-to-View-Form-Revision-History. Make sure to revert to the correct date.
