Please explain to me why I am getting Form not found?

    Your account is currently suspended. I checked your form and I can see that you asking for login credentials and credit card details. This is not allowed in JotForm. Please check our terms of use.

    If you need to collect payment from your users, you need to use one of our payment processors.
