iPhone 7 smart phone is having problems viewing the JotForm

So I have a Volunteer section on my website and I included the JotForm. http://www.utahgirlstacklefootball.com/volunteer

On my PC and MacBook laptop everything works fine. On my samsung phone everything looks great. 

Yet on my iPhone 7 it is having viewing problems. What it does is completely zooms in on a section and you can not zoom out. Then you can only see half of the form...

I did a iFrame on Wix (it worked for the Registration section). I can not figure out what to do for that volunteer section of the JotForm?

Any ideas...

I was also able to reproduce the issue on the volunteer section form here http://www.utahgirlstacklefootball.com/volunteer

However when using only the form URL https://www.jotform.us/form/63366390144153 the same issue is not happening.

I found that it has to do with the layout of the form since some of the fields where in shrink state.

I have expanded them on the form and it seems to have fix the issue.

If you scroll all the way to the problem it is not displaying the SUBMIT button? I have looked at it on other devices and just iPhone is tending to have problems.

If you have looked it to other devices, and the Submit button is not displaying ,perhaps you could adjust the height of the iFrame embedded on your site? Please adjust the iFrame height into 4894px;
