My form is missing from my website

I had recently made a form and used it on my webiste , even tried the form . now that i check the form  has been reset and missing . Why is this so ? Please correct this problem

Thank you for contacting us. If I did understand your problem correctly, you have embedded your form to your website and now the form is not showing in your website.

For further inspections, could you provide us the URL of the website?

Additionally, I would recommend you to embed your form to the website using iFrame. Please check the user guides for further information.

No that is not the problem , the problem is that the form i made on jotform
is deleted . this is the 2nd time this is happening !

I checked your account. It seems like there is only one form in your account. 

I also checked Archive and Trash folders and could not find any form.  

For additional inspections, I viewed your Account History. Please have a look at the following steps.

1. Go to your Account.

2. Click on History Segment and choose All Actions as shown in the screenshot. You have been updating the same form with ID:63281807162354. 

3. Go to Form Creations. The date of creation of your form was November 24.
