Why is my form disabled?

hello I have the problem when I was going to this link , The link prompt as "This form is currently unavailable". So, can you please help me ?

I've checked your form and it seems like it is currently disabled. You can enable it by following this guide: https://www.jotform.com/help/180-How-to-enable-disable-a-form 

However, I noticed that your form is asking for a "Pin Code". Please note that we do not allow users creating forms that ask for sensitive information like login credentials, passwords, credit card information, pin codes and the likes. This is a violation of our Terms of Use. 

This might also explain why the form might be disabled, we have an anti-Phishing system that checks form for any violations, if it finds one, then it will flag the form as spam and will suspend your account which would result to the form being disabled. 

Please do read our terms and update the form and remove fields that may contain sensitive information.
