How can I set up a form autoresponder and remove the area code part of phone number?

Additionally, I would like to tweak some items on my form and would like to speak to a representative to walk me through it. Can we setup time for a phone call?

To send an email to an address entered in the form, you would set up a form autoresponder.  Here is how to do so:

The phone number field distinguishes itself from a regular textbox by having an area code field.  If you need only a single entry for the phone number, add a textbox field to the form and label the field "Phone Number":
