Can I add a logo to my forms?

Can I add a logo to me forms that I create?

I would suggest you to use the Heading field, this will help you to display your logo as well as some heading text, first you will need to add the field to your form and click on the image option on the top toolbar, once on the wizard you will be able to provide the image by choosing on the method that you want: 

Another option would be adding the image field to your form, then choosing how you will upload the image:
