When I import my CSV file into MS Access, I get #NUM! errors.

Two of the fields are JotForm fields (submission ID and submission date).  The other two are Case # and a field that I deleted (another Date field).

I am not familiar with Access database, but following some basic guides on how to import a CSV file, I imported the one from your form, and it worked fine: http://www.ip2location.com/how-to-import-csv-into-ms-access

Please contact your database administrator, because this process is not part of our scope.
