    The solution 11474017239 displays properly in Windows 7 browsers, IE8, Firefox 10-12, and Google Chrome 18. However the pdf file does NOT display on the Mac browsers, Safari 5.17 and Firefox 12. It does display in Mac Chrome 18

    Which method are you using to embed your form? I would suggest you to use the iFrame Code

    Could you please provide us with the URL where the form is embedded so we can check?

    I would also suggest you to reset your Mac's browser settings to default.
