reservation form showed up inly one time

I got your refferal from localendar to use you there and I created a test reservation form, it worked great in an app, I used it one time and never showed up again, stills shows in the website but I closed the app, restarted and the form is not in the phone only in the website, any suggestions so it does not disapear, I am asking the same question to the localendar and the app, I think the problem is in here, welll now is not showing in the internet either, and the code is there, thanks

I did check the webpage URL which you have share but I was not able to trace any JotForm embedded there. I would suggest you to please share the webpage URL where you have embedded your form and we will take a look on why the form is not being displayed.
