Have form submissions in CSV format

    When our form is submitted can it be received as a CSV file (or any spreadsheet format)

    Unfortunately, we don't have an option to receive the submission as a CSV file. When someone completed your form, you will receive an email with a PDF attachment, if it is enabled.

    You can manually export the form submissions to a CSV file. Here's a guide: How-to-Download-Form-Submissions-as-Excel-CSV-PDF.
