How can we be "disabled" after creating only one form? I put the form on the product page template of our site. Does this mean we are charged for every product we have online? Please confirm.

Your account is currently over the form view limits. According to your forms analytics, the total looks to be correct:
If your forms are embedded to a webpage, each time the page is loaded, the form will also be loaded. Each time the form is loaded, it counts as a view. Replacing the embedded form with a link to the form will ensure that only the users who wish to fill out the form are loading it.
I reset your form view to allow some time to get things sorted. Keep in mind, we can only do one reset per account. Should your account go over limit once again, it would require upgrading or waiting for the monthly reset in order to restore your forms.
