Formulario PDF que se despliegue completo

Buenas tardes,

Al intentar pasar a PDF el formulario este se observa de la siguiente manera, es decir como los botones de SERVICIO RECIBIDO, no se encuentran habilitados no se despliega el resto del formulario, necesitaría que en el PDF el formulario aparezca completo, es decir como si todas las opciones estuvieran habilitadas.

If I understood you correctly you have issues with you Fillable PDF form. 

I have inspected your online form http://www.jotformz.com/form/62487066509666 and I saw that you have set conditions to hide and show fields. However please note that conditions will not work in Fillable PDF form. Inside of this guide you can find more about Fillable PDF:

If you want that all fields are shown on your Fillable PDF form, than I would suggest you to disable your conditions and create new PDF form.

Also from screenshot you provided I saw that you are filling your form inside of browser. You need to use Adobe Acrobat Reader or Foxit of you want to fill and submit your PDF form.
