    I assume that you would like to create a JotForm form that will have same functionality like the form you provided. 

    Before you start creating your form I would suggest you to sign up and create free account. You can use this link to sign up: https://www.jotform.com/signup 

    To create a JotForm form with same functionality like the form you provided, you will need to use conditional logic to hide and show fields. Inside of this guide you can find how to show or hide fields base on user's answer:

    You can also clone the form from webpage you provided. Some of the field will be cloned. Then you will need to add fields that are mission, probably rearrange some fields and set conditional logic to hide/show fields.
