    That happens because of a field called "spacer2"

    You will not see it in the edit mode of your form, because it is hidden through CSS:

    You also have some conditions that force it to show:

    If you do not need that field, do the following:

    1) Remove the code that hides it from the Inject CSS area:

    #id_191 {

       visibility: hidden !important;

       height: 1px;

       margin-top: 0px;

       margin-bottom: 0px;

       padding-top: 0px;

       padding-bottom: 0px;

    }

    2) Delete it from your form.

     

    3) The position the fields in this order:

    Inject this CSS codes on that form, this should correct it.

    li#id_9 {

        position: relative;

        right: 60px;

        float: right;

    }

    li#id_10 {

        clear: both;

    }
