My Form is not uploading ?

I create a form using your site, but when i copy and past the link to use it, it does not work, it says that my form was not found ?

Your account was suspended and your forms were disabled due to violation to our Terms of Use.

You cannot use our form to collect credit card information data.

If you want to use payment feature on your forms, you should use Payment integrations instead.

We can only request for consideration to reactivate your account if you comply to deleting the forms that were suspended under your account.
