    It seems that your account was autosuspended by our phishing detector, which is why your form is disabled. Your cloned form had the word "password" in its label, which our phishing detector flagged as suspicious.

    I went ahead and removed the "password" in the label of the form field to comply with Jotform's terms. I have also removed the suspension on your account, so you should now be able to use your form without any issue.
