Unable to apply a theme to one of my forms

    How can I add a theme to an existing form? I have done all of the steps of going to my theme store and hitting use theme but then when it asks me to select my form my form is not there.

    If you still face issue. Send us the screenshot of what you see on the SELECT FORM screen, so we can look into it.

    I am trying to apply this theme to one of my forms, but my form does not show up in the SELECT FORM option on the left

    It seems that you have replied to the email response received by attaching a screenshot. Please be noted that the images attached to the email cannot be posted back to the forum. We request you to access the forum thread and attach the screenshot. Please see the screenshot below:

    Also, please try logging out from your JotForm account and try logging again to apply the theme to your JotForm. Please get back to us with the form ID and theme name, if the issue still persists so that we can take a look and provide you with necessary assistance.

    In one of your first responses you mentioned that I need to change my username from guest to a different one. I replied with 3 options like requested so is it switched. The reason for my question is that was one of your answers. Saying that could be the problem of why I cannot apply the theme.

    Yes. I see that your account username is changed from guest_50323256841046 to mojo2016 with reference to the thread below.
