    How to allow the user to loop entering similar information on the form?

    Here is my problem.

    1. We need to survey our membership about possible upcoming tax reform changes.

    2. We want them to be able to select the industry they are reporting on, then the issue, and then their concerns.

    3. We want them to be able to loop through and do this for as many times as they want, but we can cap it at 4 if need be.

    4. We want them to be able to come back to the form if they cannot finish it in one sitting.

    5. We want them to be able to download a word form to use as a “scratch pad” so they can write out their answers before actually filling out the form.

    I have created the three basic pages for the form: Personal data, Industry, and comments (couldn’t see how to name the comments page since in the old builder there really is no way to easily name pages, and the page numbering seems to be one off).

    On the third page, comments, I want them to ask them if they wish to submit more comments on a separate issue. If yes, then I loop back to page 2. If no, then the form is submitted and they are sent to the thank you page.

    I just can’t seem to figure out how to do this. I have gone through the forums, but they are a mess and there is never a way to see if what applied to a problem in 2011 still applies now. And I have to use the old builder since the new one is in beta and I never know if a problem is caused by the beta or something else.

    I chose your product because it looked like it would allow me to do what I need quickly. But If it can’t, then I will move on to wufoo or another product.  I am already past the deadline for getting this done. It really sucks that there is no way I can call support. I would be willing to pay for that if it meant a quick answer. Frankly going back and forth through email is not a good way to support a product since too much time is wasted at the beginning just trying to explain the problem.

    I understand that you want to allow the user to enter information multiple times for the same field using your form. You may consider duplicating the fields on your form and display them to enter the information based on a condition. Please see the screencast below that can help you with setting up the conditions on your form.

    Since the next radio button is visible always, we can add it to the first condition to display along with the fields. Please see the screencast to change the condition further.

    Is there any way I can add text next to an image? I have the word download icon as a linked image but I need to add text to it.

    You may edit the form in the V4 editor by switching the Form builder to Version 4 in the account settings.
