Form Not Found

I am getting the Form Not Found error which could simply be an error or suspicion that my form is phising.  Could someone please look into it?  I would greatly appreciate it

Hello,

I inspected your form which you mentioned and found that a phishing field. You cannot collect such as these sensitive data via Jotform. It violates our terms of use. 

Our system detects automatically the phishing fields and suspend both forms and the related accounts. You can only use our payment wizard if you want to get payments. Please review our QA section which is related to the payment forms.

I deleted the field which includes last 4 digits of credit card and added your form in whitelist as it stands. But please note that if you want to collect sensitive data again, our phishing system'll disabled your form or forms again.
