Forms are disabled

My account got locked because i created a second account for an additional form. I will delete my other account but i need this account unlocked because im only using these forms for A FINAL TERM PROJECT FOR MY STUDENT WEBSITE. Im not profiting in anyway off of it and i will not even use the site after tomorrow. PLEASE UNLOCK MY ACCOUNT SO I CAN TURN THIS IN!!! Its not even a published website its on a local FTP.

I have checked your account that username "nathantmoss".I checked your forms. They contain "Password", "Credit Card Number" and "Expiration Date" fields. You can not directly get personal sensitive information such as password, credit card details. Because it violates our terms of use. If you insist on collecting any of this information, your account will be suspended again.  

I have deleted these fields and re-activated your account. Your form is enable now.
