    my client is a physical therapist and he wants patients to be able to fill
    out forms and then book appt. online. there might be multiple providers.

    I think you could use a drop down to show the providers, if they are multiple, this way you will list all of them so the submitter chooses which will be used: 

    Now, if there are appointments then the best option will be to display them using a widget, here is one that may help you:

    It will also allow you to set the available number of times that an option can be selected, if payments will be processed in the form then a payment integration will need to be added, here you will find them: https://apps.jotform.com/category/payment_processing 

    You will also need to manually calculate the amount due and then pass this amount to the payment field, here is a guide that will also help you to do it: How-to-Pass-a-Calculation-to-a-Payment-Field
