then on second form submit but shows submit and pagebreak?

I think that's not a problem, that's how page break works. I just checked your form and found no problem on it. Can you please add more details on your problem?
Adding 1 page break to your form would divide the form into two pages. (That displays "NEXT" button on the first page and "BACK" button on the second page.)
