Questionnaire form allows for email response based on specific answers to questions

Looking for a wp plugin - client answers multi answer questionnaire template and receives an email with specific high level "to do's" based on their answers. Can your product do this?
Quest 1 = Answer B
Quest 2 = Answer A
Quest 3 = Answer B
- - many variations of answers
