Users are getting a error message when trying to attache files to my form

Users are reporting the following message appears when trying to submit our form..

Upload Error  File could not be uploaded for some reason. Please go back and fix the problem.

This is on the SafetyNet wireless agent site compliance form.

The issue was reported immediately and our developers were able to have it fixed right away. Can you please check it on your end and let us know if you encounter any further issues.
