Same Form Still Broken, Requesting Consultation ASAP

    I am unhappy to report that the forms I was able to view again on Monday and Tuesday are no longer appearing on Wednesday. Kevin_G suggested we try the iFrame embed code. Nothing shows.

    I am requesting follow up ASAP. We have equipment that needs to be checked out and people who are looking to fill out our contest forms. Please let us know when I can speak to someone over the phone. If no one becomes available soon, we may begin to investigate other form services

    Please note that we tried to reproduce the issue to no avail. The form loads for us on all the browsers we tried.

    Still, upon inspection of your website, we found that you are not using the iFrame code yet.

    The attached screenshot shows the currently applied form code:

     

    Please consider following our colleague Kevin_G's suggestion of using the iFrame code (as shown by the tutorial here: https://www.jotform.com/help/148-Getting-the-form-iFrame-code).

    If the issue still occurs after deploying the iFrame code, please let us know so we can examine that deployment. Thank you in advance.
