None of the responsive fields are working. Please help.

I have tested a form you provided on my computer and on my phone (Samsung Galaxy S4). On both devices form is loading properly.
Also your form is responsive.
Do you still have issues with your form? If so, please provide us more details about your issue. Which responsive fields are not working? Which device are you using? If possible please provide us a screenshot of your issue.
