Every PDF attachment today is not complete.
Only the submission date is filled in. See printscreen.

I cloned your form and I was not able to replicate the issue. The attached PDF shows all the form fields. See the image below:
Please try this solution, reset the design of your PDF. Here's how:
1. In the My Forms page, select the form and then click the Submissions button.
2. After that, click the PDF button and then select Customize PDF option.
3. In the PDF Designer, please click the Reset button at the top right. After that, click the Save button.
