PDF form - poor format!

    I have created a form...https://form.jotform.com/solutio/psr-form

    When I select "Create PDF Form" the format is all messed up.

    Please advise.

    Apologies for any inconvenience. Can you try to reset the format of your PDF back to its default state and save it after?

    To do that, please follow the screenshot below:

    Supposed you are already on your form's submission page.

    Make sure the Save icon shows "Report Saved" before you try to download another PDF copy of your submission. There are also times that you need to reload your submission page (close it and load it again).

    I would like to apologize for the confusion. I see that you are referring to the "Create PDF Form" and not the Create PDF copy of submission.

    Will have to check this further and get back to you as soon as possible.

    I downloaded a Fillable PDF copy of the clone version of your form and I got the same.

    I have checked your form and I see that there are certain things that affect the design of the PDF copy of your form. One of which is the "Form Tabs widget" as it will not show in the PDF copy. Another thing is the Configurable List widgets. Widgets require scripts for them to work and are loaded from an external source that's why it may not appear in the PDF copy.

    We understand that you may need to use the PDF copy of your form especially for respondents who may not have internet connection for some reason, however, with the kind of form you have that requires collecting of data through widgets, it may not be suitable.

    If it would be possible for you to use normal form fields to substitute the Configurable List widget just for the Fillable PDF copy of your form, then I suggest you do that.
