I created my account, checked the email, did everything right, created my form and activated it, when I am going to view my own form, it says that it is disabled even though I already have activated it. How do I activate my form and appear for all See

Your account is currently disabled because of phishing, this happened because on your form you're asking for login credentials.

Please do note that asking for login credentials, credit/debit card info through plain text fields, or any other sensitive information is not allowed on JotForm, I would suggest you to take a look to our terms so you know what you can and cannot ask: http://www.jotform.com/terms/
In order to re-activate your account I have removed those guilty fields from your form, please follow this advice, otherwise your account will be permanently suspended.
