My form is behaving slowly

The form at https://form.jotform.com/51124940646150 is performing poorly.  Could you please investigate?  This just started happening today and had been working fine before.

I have tested your form and I was able to replicate mentioned issue. Your form is a bit slow. 

This is happening because of number of fields and number of conditions that your form has. Your form has 305 fields and 372 conditions. 

Although there is no limit for number of fields and conditions that you can have on your form, if there is lot of conditions it may affect of performance of the form because all conditions need to be loaded. 

I can only suggest you to reduce the number of conditions. I saw that your form has log of update calculate conditions. Maybe you can perform calculation with Form Calculation widget instead.
