My form is disabled

    I am using an Instagram Advertising form and the form has been disabled.  Please advise.

    We do not allow for the collection of usernames and/or passwords through forms for any reason.  Forms that request such data end up suspended by our phishing detector, as has yours. 

    I removed the username and password fields from the form and activated your account.  Your form is usable once again.  Please refrain from requesting such data in the future:
