Why I am getting a message of "Form not found"?

    I am having trouble generating a form, the message "form not found" appears, I am not doing anything related to pishing as I have seen in some cases, when the form is removed, I would appreciate the repair of this problem.

    I have checked your account and I have seen that your account has been auto-suspended by our automatic phishing detection system.
