I checked the page where you have embedded the form, and it is displayed:
If the issue continues, remove the form, and embed it back using the Iframe code: https://www.jotform.com/help/148-Getting-the-form-iFrame-code

Thanks. it displays fine on desltop, as yo've seen, and on ipad. But NOT on iphone 5 320 width.
It displayed correctly previously, publishing using the Muse selection, 320 width. I don't want to force using iframe code. help, please

The problem is not the form. It has to do with the style of your page, which is hiding the div where the form is embedded. You could try your form through the direct link and you will notice it displays fine in smaller views: http://www.jotform.us/form/63466182651156
Please contact your web-designer in order to fix your page.
