By default or when you integrate your form with Google Spreadsheet, it should show you the correct format of the date fields in the spreadsheet.
However, in your case, it's possible that the format was changed for some reason.
To change it back, select the column that you would like to change (please do not include the headers) then click "Format" from the spreadsheet's menubar, go to "Number", and select the appropriate format for that specific column.
You can also try to reintegrate (remove integration from present integration and integrate again) your form with Google Spreadsheet using the same Google Account. It would, however, become a different spreadsheet already and connections with earlier integrations will be lost.
