I have set up the form but it won't score, any tips please?

I found submissions in your form with 0 scores despite the fact the based on your formula for the field Total Score, they should not get 0. What I noticed is that they usually come in succession - 2 or 3 in a row. Please see below.
I have suspected that such scenario is possible if you submit the form more than once (by filling up the form, clicking submit, clicking Back button in the browser and submitting the form again). Please see below for the scenario.
Did your customers with 0 total score encounter any error during their submission? Or the scenario I have given above happened to them?
