Can someone look into this kind of quickly I need to get some forms out to a client and I am not sure why this is saying this

I see that your account is having a form titled "Nathan Benner" is collecting Credit card information using plain text fields which is violating our terms of usage. and so the account is suspended by our anti-phishing system. If you need to collect payments with JotForm, you may consider using one of the payment fields from the form builder.
You may also enable the Authorization only option for PayPal, Stripe and Authorize.net so that you may charge the customer later from the payment gateway dashboard.
Your account can be activated only if you agree our terms of usage. Please confirm that the guilty fields on the form will be deleted,

don't think she understood how to use the form. Did you enable it again? t

button so I am not sure what happened but if you are not going to release
this then I need to know because I have to move on to a different form
provider

Unfortunately , we do not allow forms that request credit card information on plain text fields. I know you sent to us an email explaining that you only request the latest 4 digits, yet this practice is not allowed. I also know that you cloned this from our form gallery. Our users are able to share these type of templates and we keep cleaning the bad templates, but they sometimes slip from our control, sorry for the confusion.

authorization? I don't want to take payments just use this as their booking
forms and cc auth forms. thanks

As mentioned earlier, you may enable the Authorization only option from the payment field. You may charge the user later from the payment gateway if necessary only. Please let us know if this method works for you.

actually ever take payments they go through the supplier so that is not an
issue.
