How do you enable the form?

    The form says it's disabled but it's not. How do you enable it?

    Our phishing detector tool auto-suspended your account because you were asking for credit card information, it is against our terms of use. Please remove all the fields asking for credit card information on both of your form before we re-activate your account. See image below:
