Please help me with this problem. I've been stuck in the help forum for weeks trying to figure this out. It seems like a glitch in my form but I don't know how to fix it. I've deleted the heading and re-added it and that didn't work.
When I look at my submissions I can see all my headers. And I checked the box for 'show headers and text'. When I click on the 'get pdf' button- most of my headers show... but not the first (and most important) one.
Why are some showing and others are not? My screenshot shows what I mean.

I understand your point, this is the screenshot I get from my test (I cloned your form)
On the submission view, the first Header appears, but when I open the PDF file, it is missing
So, I am forwarding this to 2nd level in order to investigate and rectify this

I cannot provide you with a timeframe for when 2nd level will update on this thread, but as far I as can on the status of the ticket the issue is already given an Important Priority. So I think they should update on this soon

Our apologies but the problem isn't yet resolved. However, I have increased the priority of this issue so our development team will be notified about this. We will update you here once the problem is fixed.
For the meantime, try replacing your first header field with Free Text HTML and use the following code below to create a header style.

I tried the suggestion of using the Free Text field instead of header field. It didn't work. It still doesn't show up in the PDF and when you view the regular submission area it shows that code which is not ideal as it looks sloppy to the user. Please keep working on this and let me know if there is something else to try.

Hi, are you referring the compound Address field?
If you set this field to *required, all the individual fields will become required. The only way you can have an individual requirement for each is, by using the regular textbox fied and create the fields for each.
