Can't enable new form

    Hi - new user here and have just created my first form. However, I can't enable this form. If you look at the attached screen showing preferences, no matter how often I change them they changes don't stick.

    I checked your account and it was suspended by our system for suspected phishing.  Though your forms are within our terms of use, there were several terms in the forms that set off our phishing detector.  Terms like Visa (usually related to credit cards) and passport (which we manually review forms that ask for such data) when used together can raise the phishing score for the form high enough to set off our system.

    I whitelisted your form and it should no longer cause any trouble.  Your form and account are active once again.
